急性膵炎における副腎の造影効果亢進とその重症化予測能：『急性膵炎診療ガイドライン』との関連について

Predictive value of adrenal gland enhancement for the severity of acute pancreatitis：association with “Japanese Guidelines for the Management of Acute Pancreatitis”

外傷による循環血漿量減少性ショックや敗血症性ショックの症例において，ダイナミック造影CTで副腎の造影効果が亢進し，これが予後と関連することが以前より知られている^1）2）。近年では急性膵炎^3）やICU入室患者^4）5），急性腸間膜動脈閉塞症^6）でも同様の現象が報告されている。この現象は，急性の強い生理的ストレスに対する交感神経系ならびに視床下部-下垂体-副腎（hypothalamic-pituitary-adrenal：HPA）系を介した応答を反映したものと推定されている^1）。

In cases of acute pancreatitis, the relationship between the adrenal enhancement on the arterial phase of dynamic contrast-enhanced CT and severity assessment based on the “Japanese Guidelines for the Management of Acute Pancreatitis” was investigated. As a result, increased adrenal enhancement in the arterial phase of dynamic contrast-enhanced CT in acute pancreatitis may serve as a rapid imaging biomarker. It may be useful for identifying patients at risk of deterioration among those classified as mild according to the guideline-based assessment.
