画像診断が鑑別診断に有用であった陳旧性長内転筋断裂の1症例

A case of old adductor longus rupture in which imaging findings were useful for the differential diagnosis

大腿部の筋損傷は軽度のものから完全断裂をきたすものまで様々な病態があり，また発症部位や誘因も多様である。長内転筋断裂は大腿部の内側に力がかかる運動，負荷などで生じるとされ，筋損傷に伴う腫瘤形成や鼠径部の疼痛，違和感として自覚されるまれな疾患である。典型例では特徴的な発症機転や症状，臨床経過から容易に診断可能であるが，陳旧例で特徴的な臨床経過が不明瞭な場合には診断が困難で，筋損傷による腫瘤が鼠径ヘルニアや軟部腫瘍と診断されてしまうことがある。今回我々は，陳旧性で特徴的な症状や臨床経過を示さず，画像検査が診断に有用であった長内転筋断裂の症例を経験したため，若干の文献的考察を加えて報告する。

We report a case of old adductor longus rupture in which the diagnosis was made based on computed tomography（CT）and magnetic resonance imaging（MRI）findings. Adductor longus rupture is a rare disease and is often diagnosed based on characteristic symptoms and clinical course. However, in chronic cases, the symptoms and trauma history can be unclear, making it difficult to determine a diagnosis based on medical history and physical examination alone. Old adductor longus rupture has characteristic CT and MRI findings that can facilitate diagnosis in cases in which symptoms and trauma history are unclear.
