Inflammatory disease of the liver

肝に発生する代表的な炎症性疾患として，肝膿瘍や寄生虫感染症，肝肉芽腫症，炎症性偽腫瘍などが挙げられる。これらの疾患では，臨床症状や検査所見が非特異的であることも多く，画像診断は診療において重要な役割を担う。一方，肝炎症性疾患の中には悪性腫瘍に類似した画像所見を呈するものも少なくなく，鑑別に難渋することがある。したがって，各疾患の典型的な画像所見を理解し，炎症性疾患を鑑別に挙げる視点をもつことが重要である。本稿では誌面の都合上，肝炎症性疾患のうち日常臨床で遭遇する頻度が比較的高い肝膿瘍ならびに肝炎症性偽腫瘍について概説するとともに，悪性肝腫瘍との鑑別のポイントについて解説する。

This article focuses on hepatic abscess and inflammatory pseudotumor（IPT）, among inflammatory liver diseases, and reviews their imaging features with an emphasis on differentiation from malignant tumors. These entities often present with nonspecific and diverse imaging findings that may mimic primary or metastatic hepatic malignancies, making accurate diagnosis challenging. Characteristic imaging features, together with clinical information, are essential for appropriate diagnosis. In addition to imaging findings of hepatic lesions, extrahepatic manifestations, patient background, clinical symptoms, and laboratory data should be comprehensively evaluated. A multidisciplinary approach is crucial to avoid misdiagnosis and unnecessary invasive treatment.
