骨盤内巨大動静脈奇形に対し大動脈遮断バルーン併用経動脈的塞栓術を施行した1例

A case of a massive pelvic arteriovenous malformation treated with transarterial embolization under aortic balloon occlusion

動静脈奇形（arteriovenous malformation：AVM）の侵襲的治療としては，血管内治療あるいは血管内治療を併用し根治的切除術が施行されることが多い。血管内治療においてはnidusのみを塞栓するために，flow controlや流速に合わせた塞栓手順，塞栓物質の選定が肝要である。今回我々は，通常の手法ではflow controlが困難であったが，大動脈遮断バルーンを用いることにより良好なflow controlが得られ，経動脈的に塞栓に成功した骨盤内巨大AVMの1例を経験したので，文献的検討を加えて報告する。

We report a case of a massive pelvic arteriovenous malformation（AVM）that was successfully managed through transarterial embolization under aortic balloon occlusion, resulting in a favorable outcome without complications. In the treatment of giant pelvic AVM, where multiple inflow and outflow pathways render flow control with only a balloon catheter challenging, aortic balloon occlusion can serve as an effective adjunctive technique.
