腎血管筋脂肪腫（angiomyolipoma：AML）は日常診療で遭遇する比較的頻度の高い疾患である。2006年，AMLの特殊型として内部に上皮性嚢胞を伴うangiomyolipoma with epithelial cysts（AMLEC）という亜型が提唱された^1）2）。AMLECの画像所見に関しては症例報告が散見されるが，まとまった報告はなく，ときに腎細胞癌との鑑別が問題となる。AMLECの画像に関する経時的変化の報告はない。今回，増大傾向から最終的に切除に至ったものの経過の画像にて発生過程を観察したAMLECの1例を経験したので報告する。

45 years old woman was found to have a right renal mass. The lesion initially appeared as a simple cystic mass and developed into the mass with thickened wall and mural nodule. Preoperative CT and MRI showed solid mass with marginal collapsed cyst and in which the solid lesion showed hypointense on T2-weighted MR images and hypervascular on contrast enhanced CT. The renal mass was finally diagnosed as angiomyolipoma with epithelial cysts, rare subtype of renal angiomyolipoma.
