嚢胞変性を伴う縦隔腫瘍と鑑別を要した肺過誤腫の1例

A case of pulmonary hamartoma mimicking a mediastinal cystic lesion

肺過誤腫は最も一般的な良性肺腫瘍である^1）。ポップコーン状の石灰化や脂肪成分が特徴的とされるが，50％の症例では石灰化も脂肪成分も認めずしばしば診断に苦慮する^2）3）。今回我々は，石灰化も脂肪成分も認めず，嚢胞変性を伴う縦隔腫瘍との鑑別を要した肺過誤腫の1例を経験したので文献的考察を加えて報告する。

We report a case of a 58-year-old female with a pulmonary hamartoma mimicking a mediastinal cystic lesion. Characteristic imaging findings of pulmonary hamartoma include not only calcification and fat but also a mass with septal-like contrast enhancement within the mass, subtle contrast enhancement, hyperintensity on T2-weighted images, and high ADC values. Even if it appears to be a tumor of mediastinal origin at first glance, when these findings are observed, the possibility of a pulmonary hamartoma should be considered.
