Duplicated origin of MCAを有した内頸動脈閉塞に対して血栓回収療法を行った1例

A case of thrombectomy for the internal carotid artery occlusion with the duplicated origin of MCA

脳主幹動脈閉塞の血栓回収療法においてステントリトリーバーを使用する際には閉塞部血管より遠位へマイクロカテーテルを誘導する必要があるが，脳血管撮影上で描出されない閉塞血管であるため，閉塞遠位の血管走行を予想しマイクロワイヤーおよびマイクロカテーテルを誘導する必要がある。しかし，閉塞部主幹動脈に正常破格がある場合，一般的に予想されない血管の蛇行や口径の狭小化が存在する場合があり，治療するうえで注意が必要である。今回我々は，duplicated origin of MCA（中大脳動脈）を有する左内頸動脈閉塞に対して血栓回収療法を行った症例を経験したので文献的考察を加え報告する。

We present a case of mechanical thrombectomy performed on a 97-year-old female patient who experienced a rare anatomical variation with a duplicated origin of the middle cerebral artery（MCA）. The patient was admitted to our hospital following a sudden loss of consciousness. Imaging studies revealed an acute cerebral infarction caused by occlusion of the left internal carotid artery. Emergency thrombectomy was carried out around 23 hours after symptom onset. The procedure was performed on both MCAs arising from the duplicated origin, and successful recanalization was achieved in 3 passes.
