心尖部心室瘤と冠動脈左室瘻を認めた肥大型心筋症の1例

A case with hypertrophic cardiomyopathy, left ventricular aneurysm, and multiple coronary artery–left ventricular fistulae

肥大型心筋症（hypertrophic cardiomyopathy：HCM）は心肥大に基づく左室拡張能低下を特徴とする疾患群である。HCMは左室瘤を合併することが知られているが，冠動脈左室瘻を合併することはまれである^1）。今回我々は，心尖部心室瘤と冠動脈左室瘻を合併したHCMの症例を経験した。マルチモダリティを使用することにより，非侵襲的に診断可能であったため，文献的考察を加えて報告する。

Hypertrophic cardiomyopathy（HCM）is a disease characterized by reduced left ventricular diastolic function due to cardiac hypertrophy. Some patients with HCM develop a left ventricular aneurysm due to an increased pressure load on the apex caused by central obstruction during the course of the disease. However, coronary left ventricular fistulas with HCM are rare. We report a case of apical left ventricular aneurysm and coronary artery‒left ventricular fistula in a patient with apical HCM using multimodality non–invasive imaging.
