閉塞性黄疸に対する経皮的胆管ドレナージ（percutaneous transhepatic bile duct drainage：PTBD），胆管ステントは歴史の長い手技であるが，内視鏡的胆道インターベンションの隆盛とともに，件数は減少傾向である。しかし，確実な外瘻が必要な症例や内視鏡到達困難例など，経皮的手技のみにて治療可能な症例も存在し，技術を継承していくことは非常に重要である。超音波手技や経皮的穿刺手技に長けた放射線科医はその担い手として適当と考えられ，より多くの先生方に胆道系IVRに参画いただきたいと考えている。本稿では主にPTBDにおける知っておくと役に立つtipsを概説する。

Percutaneous transhepatic bile duct drainage（PTBD）and bile duct stenting for obstructive jaundice are historic procedures, but their number is decreasing with the rise of endoscopic biliary interventions. However, there are cases that can be treated only with percutaneous techniques, such as those requiring secure external fistulas or those difficult to reach by endoscopy, and it is very important to pass on these techniques to future generations. To encourage more radiologists to participate in IVR of the biliary system in the future, this article outlines tips that are useful to know mainly in PTBD.
