化学放射線療法により10年生存が得られた食道癌術後リンパ節再発の4例

Four cases of postoperative lymph node recurrence of esophageal cancer with long-term survival achieved by chemoradiotherapy

我が国における食道癌術後再発率は28～47％である^1）。その再発様式は，局所領域再発が最も多く22～68％，次いで遠隔転移12～51％，両者の複合が7～27％である^1）。食道癌術後再発に対する治療は，再発様式・進展範囲に影響を受けるが，患者の全身状態，患者・家族がどれだけの有害事象を許容するかも考慮する必要がある。これらに対して集学的な救済治療が実施されているが，再発診断からの生存期間の中央値（median survival time：MST）は5～10カ月であり，食道癌術後再発の予後は極めて不良である^1）。

We report four cases of long-term survival of more than 10 years after initial treatment using chemoradiotherapy for postoperative lymph node metastasis of esophageal cancer without distant metastasis. The subjects were 4 patients who underwent chemoradiotherapy at our hospital between April 2010 and July 2012. The ratio men to women is 3：1, the age distribution ranged from 61 to 67, 1 patient with stage Ⅰ and 3 with stage Ⅲ. All patients were treated with image-guided intensity-modulated rotational irradiation with simultaneous CDGP/DOC. The survival period ranged from 123 to 145 months, one patient survived with cancer, and three patients are disease-free without no treatment.
