CTガイド下肺生検（CT-guided needle biopsy：CTNB）はCT装置の普及により広く行われているinterventional radiology（IVR）手技の一つである。手技に伴う合併症は気胸や肺内出血による喀血，血胸，空気塞栓などが挙げられる。このうち血胸は自然に止血が得られることが多く，重篤な血胸をきたす頻度は0.1％とまれであるが，出血性ショックや呼吸不全により，死亡例も報告される。今回，CTNB後に血胸による出血性ショックをきたしたが，肋間動脈塞栓術を行い救命し得た1例を経験したので報告する。

Computed tomography guided needle biopsy（CTNB）is one of interventional radiology technique, which have been widely used with increased access to CT. The major complications in CTNB for a pulmonary lesion are pneumothorax, hemoptysis（due to alveolar hemorrhage）, hemothorax, and air embolism. Although the incidence rate of massive hemothorax after CTNB is reported as low as 0.1％, the result can be serious including death of patient. We report the case of massive hemothorax after CTNB for lung, which was rescued by trans-arterial embolization.
