感染性大動脈瘤ならびに十二指腸への膿瘍穿破に伴う消化管出血を呈した化膿性脊椎炎の1例

A case of pyogenic spondylitis with infected aortic aneurysm and duodenum ulcer

化膿性脊椎炎と感染性大動脈瘤の合併はまれであるが，さらに化膿性脊椎炎に伴う椎間板膿瘍の十二指腸穿破による消化管出血を合併した報告はこれまでにないため，若干の文献的考察をふまえて報告する。

A 50’s-year-old male presented with complaints of fever, vomits, back pain, and both lower limbs pain. CT and MRI revealed pyogenic spondylitis. After admission, hematemesis was observed and endoscopy revealed duodenum ulcer, which had turned out to communicate with intervertebral disc abscess via the fistula. When hematemesis recurred 10 days later, CT detected infected aortic aneurysm due to pyogenic spondylitis and emergency operation was performed. Although he improved temporarily, he died of sepsis 64 days after admission.
