肺腺癌術後早期に小細胞癌として再発した1例

A case of recurrence as small cell carcinoma early after lung adenocarcinoma surgery

肺腺癌術後に小細胞癌として再発することがあるが，ほとんどがEGFR-チロシンキナーゼ阻害薬（tyrosine kinase inhibitor：TKI）加療後であり，薬剤抵抗性獲得の機序の一つとして小細胞癌への形質転換が報告されている^1）。頻度としては，EGFR-TKIを使用された患者のうち，3～14％との報告があり^2）3），比較的まれと考えられる。今回，薬物療法未施行にもかかわらず，肺腺癌術後早期に小細胞癌として再発した1例を経験したため，報告する。

Recurrence of small cell lung carcinoma（SCLC）after adenocarcinoma surgery has been reported as a transformation to SCLC as one of the mechanisms of drug resistance, mostly after treatment with EGFR tyrosine kinase inhibitors. We herein report a case of recurrence as SCLC in the early period after lung adenocarcinoma surgery, despite the lack of drug therapy.
