Ⅰ．はじめに

　頭蓋内テント上病変により生じた下向性テント切痕ヘルニアにより病理学的変化の1つとして後頭葉の梗塞を生ずることは，Meyer^7）（1929）の報告以来知られていることである．しかし，テント切痕ヘルニアの臨床症状として同名側半盲を認めた報告^{2,4,5,8,11）}は比較的少ない．

　筆者らはテント切痕ヘルニアより回復した急性外傷性頭蓋内血腫例の術後に，同名側半盲を確認した2症例を経験し，5年から7年間の経過観察を行った．この視野障害の特徴を示すとともに，1症例について術後早期に繰り返し施行した脳血管写と術後7年目のCT scanの所見より，その発生機序に関して興味ある知見を得たので文献的考察を加えて報告する．

Although it is well-known that the postmortem examination of patients who died of downward transtentorial herniation not rarely shows an infarction in the occipital lobe. Clinically, however, not many cases have been reported who manifested homonymous hemianopsia after survival from the herniation.

This report concerns with such two cases
