CTが診断に有用であった帯状疱疹罹患後の偽性腹壁ヘルニアの1例

Imaging diagnosis of postherpetic pseudohernia：a case report

帯状疱疹後運動神経麻痺として，顔面神経が侵されるRamsay-Hunt症候群がよく知られているが，まれに脊髄神経を侵し，偽性腹壁ヘルニアを生じることがある。偽性腹壁ヘルニアによる腹部膨隆は，皮膚科ではよく知られた病態であるが，皮膚科以外の診療科での認知度は高くなく，原因不明の腹部膨隆として腹壁ヘルニアや軟部腫瘍が疑われて画像診断の対象となることがある。今回，CTで診断し得た帯状疱疹罹患後に生じた偽性腹壁ヘルニアの1例を経験したので，診断に有用と思われる画像所見について文献的考察を交えて報告する。

A 70-years-old man became aware of right abdominal bulge after herpes zoster in the right Th11 dermatomes. Computed tomography（CT）revealed no abdominal muscle defects and showed thin external oblique and internal oblique, transverse abdominis, anterior rectus abdominis. We diagnosed it as postherpetic pseudohernia. Postherpetic pseudohernia shows a clinical feature similar to abdominal wall herniation. Therefore, we need to consider the possibility of postherpetic pseudohernia when we see a patient with abdominal bulge.
