A man in his 60 s developed left-sided pleural effusion secondary to a pancreaticopleural fistula. The pleural effusion could not initially be attributed to a pancreatic etiology;however, computed tomography revealed a fistula, which improved following appropriate management. Diagnosis of pancreaticopleural fistula based on clinical presentation is challenging in patients without any abdominal symptoms. Clinicians should consider this condition as a contributor to pleural effusion and perform imaging tests to promptly diagnose the fistula in patients with such a presentation.
