呼吸器症状で発症した膵胸腔瘻の1例

Pancreaticopleural fistula presenting with respiratory symptoms：a case report

膵胸腔瘻は慢性膵炎に合併することが多いとされ，膵管の破綻により形成された仮性嚢胞が縦隔へ進展し，胸腔穿破して血性胸水貯留をきたすまれな疾患である。今回我々は，呼吸器症状で受診し胸部X線で左胸水貯留を認め，画像検査で瘻孔の存在を指摘することで膵胸腔瘻の診断に至った症例を経験したため報告する。

A man in his 60 s developed left-sided pleural effusion secondary to a pancreaticopleural fistula. The pleural effusion could not initially be attributed to a pancreatic etiology；however, computed tomography revealed a fistula, which improved following appropriate management. Diagnosis of pancreaticopleural fistula based on clinical presentation is challenging in patients without any abdominal symptoms. Clinicians should consider this condition as a contributor to pleural effusion and perform imaging tests to promptly diagnose the fistula in patients with such a presentation.
