Excessive alcohol consumption is a major cause of both acute and chronic pancreatitis. A recent nationwide epidemiological survey showed that alcohol drinking accounted for 31.4% of the patients with acute pancreatitis and 64.8% of those with chronic pancreatitis in Japan. The mechanisms responsible for alcohol-associated pancreatic injury remain to be established, but several hypotheses such as alcohol-toxic theory and protein-plug theory have been proposed. Because only a small portion of heavy drinkers develop pancreatitis, multiple factors such as smoking and genetic background are involved in the pathogenesis of alcohol-associated pancreatitis.
