化膿性脊椎炎は椎体や椎間板を主座とする近年増加傾向の感染症である。その好発部位は腰椎であり，胸椎発生は14.3％程度である^1）。特に上位胸椎発生は中下位胸椎と比較して少なく，非典型的とされる^2）。また，化膿性脊椎炎から胸腔への感染の進展は非常にまれである^3）。今回我々は，画像上椎体浸潤を伴う肺癌との鑑別が問題となった右上葉への炎症波及を伴う化膿性脊椎炎の症例を経験したので報告する。

Pyogenic spondylitis is an infrequent infection, but its incidence is increasing and its diagnosis is often difficult. We report a case of a men in his 40 s with pyogenic spondylitis with pulmonary involvement mimicking spinal invasion of right lung cancer. Pyogenic spondylitis should be considered when pulmonary mass with spinal destruction is enlargement and shrinkage in a relatively short period on CT.
