悪性リンパ腫の40％程度が節外に発症するとされる。腹部領域では消化管が多く，肝や膵原発の症例の報告もある。胆道系ではリンパ組織を欠くもしくはわずかとされており，胆道系を原発とする悪性リンパ腫の報告は極めてまれである。今回，胆嚢と三管合流部胆管の壁肥厚，膵腫大から，自己免疫性膵炎に合併した胆嚢癌と胆管癌を疑い，手術を施行後，病理組織学検査にて濾胞性リンパ腫と判明した1例を経験したので，文献的考察を加えて報告する。

We report a case of primary follicular lymphoma of the biliary tract in an 82-year-old male. CT demonstrated the thickening smooth wall of the extrahepatic bile duct and gallbladder with homogenous enhancement. On MRI, the gallbladder wall showed high signal intensity on the T2-weigthed images. Intrahepatic and extrahepatic bile ducts were normally seen on MRCP without bile duct strictures. The resection of gallbladder and extrahepatic bile duct was performed. The histological examination diagnosed follicular lymphomas.
