DLBとADの鑑別診断における脳血流SPECT上のcingulate island score偽陰性，偽陽性例の検討

The characteristics of AD and DLB with pseudo-positive and pseudo-negative of CIScore.

レビー小体型認知症（dementia with Lewy bodies：DLB）はアルツハイマー型認知症（Alzheimer’s disease：AD）に次いで多い変性性の認知症疾患である^1）。DLBはパーキンソン症状，幻視，レム睡眠障害，うつ症状などの多彩な自覚症状，他覚症状を呈し^1），治療としてはコリンエステラーゼ阻害薬や非選択的NMDA受容体遮断薬が有効である一方^2），抗精神病薬に対してはパーキンソン症状の悪化などの副作用があることから，早期に，かつ正確に鑑別診断をする必要がある。しかし臨床症状のみからの鑑別診断は困難な場合もあり，画像診断がその一助となる^3）4）。FDG-PETを用いた研究で，DLBではADと異なり後部帯状回の糖代謝が保たれる，いわゆるcingulate-island sign（CIsign）を認めることが知られている^5）6）。Imabayashiらは脳血流SPECT検査を用いた検討で，同様にDLBでは後部帯状回の脳血流が保たれる現象を基にCIScoreを考案し，臨床現場で用いられている^3）4）。しかしながら，実際の臨床現場では一定の割合で偽陰性や偽陽性の症例を経験することもあり臨床症状とも乖離する場合がある。本研究ではCIScore上は偽陰性および偽陽性を呈するAD，DLBの特徴をそれぞれ検討し考察した。

We investigated the characteristics of patients with AD showing pseudo-positive and cases with DLB showing pseudo-negative when we diagnose by the value of CIScore. The AD cases with the false-positive in the CIScore had higher MMSE values compared with those of the true-negative cases. The false-negative DLB patients by the value of CIScore were older than true-positive DLB cases. When we differentiate DLB from AD by the values of CIScore, we should take into account of patient’s age and MMSE score.
