QUAD SHOT変法が奏効した頭頸部癌の2例

Two cases of advanced head and neck cancer treated with modified “QUAD SHOT”

頭頸部癌患者において全身状態不良例，放射線治療や放射線化学併用療法に伴う長期の治療期間や有害事象に耐えられない症例，高度に局所進行しており，根治照射が難しい症例，遠隔転移を有する症例などでは根治的照射の適応となりにくい。そのような患者においても，嚥下困難や嗄声，疼痛，出血などの症状を有する場合は緩和的放射線治療が有効とされている^1）2）。

“QUAD SHOT” is the cyclical hypofractionated palliative radiotherapy regimen available for advanced head and neck cancer. This regimen consists of 14-14.8Gy in 4 fractions over 2 days, given twice a day. If there is no tumor progression, this may be repeated every 3 to 6 weeks for a total of 3 courses. Two patients who were treated with modified “QUAD SHOT” obtained relatively good symptom palliation and tumor regression with no acute toxicity.
