胸部悪性腫瘍に対する化学放射線療法に伴う心臓合併症の画像所見

Imaging findings of the chemoradiation-therapy related cardiac complications for thoracic malignancies

胸部悪性腫瘍に対して，化学療法と放射線療法を併用する化学放射線療法（chemoradiotherapy：CRT）が広く行われており，主に乳癌，肺癌，食道癌，悪性リンパ腫が対象疾患である。化学療法の進歩，放射線治療技術の向上，比較的早期の症例への適応の拡大等により長期生存が期待できるようになった。それに伴い，化学療法に用いられる薬剤による心臓疾患（薬剤性心臓疾患），放射線照射野内に心臓が含まれることによる心臓疾患（放射線関連心臓疾患）が長期生存例で問題となっている（図1）。両者は互いに増悪の危険因子であり合併することも多く，本稿では以下 “CRT関連心臓疾患” と記述する。今回，CRT関連心臓疾患について概説し，それらの画像所見と画像を用いた早期検出と予防の可能性について述べる。

Chemoradiotherapy（CRT）for thoracic malignancies may give raise cardiac complications. For the evaluation of CRT-related cardiac complications, functional imaging as well as biological marker have an advantage in the detection of an early myocardial damage due to CRT. In this review, we demonstrated the imaging findings of the CRT-related cardiac complications and discuss the role of the imaging modalities in the prediction of CRT-related cardiac complications.
