要旨
目的:測定条件A(素肌)、B(1mm程度の袖の上)、C(2mm程度の袖の上)での血圧測定の妥当性を検証する。方法:被検者は、健康な女子100名および都内の介護老人保健施設に入所している健康問題を持った65歳以上の男女老年者100名である。各々の被験者に3種類の測定条件(各3回ずつ計9回)を無作為に割り付けて比較する層別無作為化比較実験とした。測定条件A・B・Cの条件下で、血圧をそれぞれ3回測定して平均値を比較した。結果:健康な女子は、①測定条件A、B、Cによる血圧値の比較を行なった結果、収縮期血圧A、B、Cの差は認められなかった。②測定条件A、B、Cによる血圧値の比較を行なった結果、拡張期血圧A、B、Cの差が認められた。しかし、相関比η=0.194と差は小さかった。健康問題を持つ老年者は、①測定条件A、B、Cによる血圧値の比較を行なった結果、収縮期血圧、拡張期血圧ともに差は認められなかった。②老年者の合併疾患については、各個人の現病歴のうち循環器機能に影響を及ぼす疾患の有無と測定条件A、B、Cによる血圧値の比較を行なった結果、収縮期血圧および拡張期血圧A、B、Cとの関連はみられなかった。
Abstract
Purpose: We investigated the accuracy of measuring blood pressure with three measurement stages consisting of A(bare skin), B(through 1 mm clothing) and C(through 2 mm clothing).
Methodology: The subjects included 100 healthy adolescent girls and 100 elderly men and women aged over 65 years with health concerns who were placed in a healthcare facility for seniors in Tokyo. The method employed was stratified randomization comparison where each subject was randomized to three measurement stages(3 times each for a total 9 measurements each) to compare the results. Blood pressure was measured three times each for stages A, B and C and the average values were compared.
Conclusion: For the healthy adolescent girls, a comparison of the results for each of the three measurement stages revealed no differences in systolic blood pressure but differences in diastolic blood pressure. However, the correlation ratio showed a small difference of 0.194. For the elderly with health problems, 1)no difference was observed in either the systolic or diastolic blood pressure, based on comparing the results from each of the three measurement stages, and 2)no correlation was observed between the complications experienced by the elderly with health problems and either systolic or diastolic blood pressure in the three measurement stages, based on comparing blood pressure measurements in the three measurement stages and the presence or absence of a disorder influencing cardiovascular function in the medical history of each subject.
