This study aimed to develop the Kanazawa and Kochi Disaster Preparedness Check Sheet, with which ventilator-assisted homecare individuals and family caregivers can prepare for disasters with homecare medical staff. First, the following items were selected from a preceding study for inclusion in the check sheet: 1. attributes, family caregivers, key person and living environment 2. disease name, care requirement category, disability level category, ADL, medical procedures and prescriptions, 3. material stockpile 4. evacuation behavior, and 5. disaster management support system. Then, 5 groups of homecare individuals, family caregivers, and homecare medical staff used the sheets and discussed what preparations they had made and what support they would need. The families in all 5 groups responded that they would never want to evacuate and that they had not prepared any equipment in their house. These results suggest that it might be necessary to add some blank space to write down whether they require assistance from professionals or from the local or the national government for support, and to describe how they feel about evacuation.
