グループ作業によるヒステリー性精神病の回復過程

Recovery process for a female patient with hysterical psychosis in the group occupational therapy

要旨：ヒステリー性精神病者の作業療法を通して，グループ作業の治療的意味と回復過程について検討した．「手工芸グループ」という女性のみのグループに参加させ，手工芸を行うことによって，グループは同性・同世代の患者と健康的な友人関係を作る促進剤となった．作業療法導入時には，セラピストは治療的退行の促進と退行できるような活動を選択し，セラピストとの治療的信頼関係が確立された後は支持的に関わり，治療的退行を促進するような活動を避けることが必要であると考えられた．そして，周囲への関心が芽生えたときに，セラピストは適切な助言を与え，慣れた活動を提供することによって満足感や有能感を味わうことができると考えられた．

　The purpose of this article is to discuss the therapeutic meaning of group activity and the recovery process in a female patient with hysterical psychosis. She was involved in the craft group and made handicrafts. As a result, the craft group promoted the friendships in that she made friends with patients of the same sex and age. It is necessary for the occupational therapist to increase the therapeutic regression and select the activity to increase regression at first. After the patient established the therapist-patient relationship, the occupational therapist supported the patient. Now it is necessary for the occupational therapist to avoid the promotive activity of the therapeutic regression. When the patient shows signs of interest in others, the occupational therapist provides the appropriate advice and offers the successful handicraft activity as an example of her ability. So the patient can experience satisfaction and capability.
