Abstract Objective : The purpose of this study was to examine the application of a clinical pathway (CP) for the local stroke network in the Kitakyushu medical care zone and to confirm improvement of functional outcomes. Subjects and methods : Subjects were 454 patients who received medical treatment in an acute care hospital and rehabilitative treatment in a convalescent rehabilitation hospital, and whose CP forms for each hospital were sent to the CP office via the Kitakyushu City Medical Association. The subjects were divided into 2 groups : the 2009 group (registered from April, 2009 to March, 2010) and the 2010 group (registered from April, 2010 to March, 2011). Results : The mean age of the subjects was 74.0±11.5 years, and the most common type of stroke was cerebral infarction for 62.6%. Six acute care hospitals and six convalescent rehabilitation hospitals used CP forms in 2009, and 18 and 28 hospitals used CP forms in 2010, respectively. There were no significant differences in Barthel index (BI) scores, BI efficiency, and length of hospital stay between the two groups. The rate of discharge to home decreased in number in 2010. Conclusion : The CP is spread throughout the secondary medical care zone as well as the primary medical care zone of Kitakyushu City. The functional outcomes and the rate of discharge to home did not improve for one year.
