Abstract : Objectives : We suggest a set of issues that were revealed in establishing a network of convalescent rehabilitation hospitals to access the functional prognosis of patients at discharge from the standpoint of dealing with rehabilitation in an acute hospital. Methods : Out of 819 stroke patients who used the liaison critical pathway, 627 (406 men and 221 women) who sent replies were enrolled in this study. Physical and cognitive function at transfer, improvement while in the convalescent rehabilitation hospital, and final outcome were investigated. Results : Time to transfer to a convalescent rehabilitation hospital was 32.2±15.1 days. Mean length of stay in a convalescent rehabilitation hospital was 97.5±59.4 days. Patients with higher scores on the modified Rankin Scale (mRS) were older and they had lower scores for functional independence measure (FIM) caused by severe neurological deficits. There was an obvious difference in mean length of stay and ADL improvement per day (FIM efficiency) among the referred medical districts. Conclusion : All healthcare facilities using the liaison critical pathway need to have a common understanding of stroke rehabilitation. By using the liaison critical pathway network system, rehabilitation staff in acute hospitals should proactively contribute to the qualitative improvement of stroke patients' rehabilitation during the recovery period.
