北海道・札幌市の地域連携パスの現状

　札幌市では大腿頚部骨折地域連携パスは1つのみが運用され，脳卒中地域連携パスは2つ運用されている．大腿頚部骨折のパスは2007年6月から運用が開始され，その後に札幌市大腿骨近位部骨折地域連携パス協議会が結成されてそれを運用している．脳卒中の方は札幌市脳卒中地域連携パスネット協議会（以下，札幌脳卒中パス協議会）が結成され，2008年4月からエクセルを利用したパスを運用しているが，ファイルメーカーを利用したもう1つのパスも2009年から併用運用をパスネット協議会が認め北海道広域医療連携協議会が中心になり運用している．

　この札幌脳卒中パス協議会のパスが北海道のいくつかの地域で使われているが，そのほかのパスを独自に使っている地域もある（図1，表）．

Abstract : In Sapporo, the clinical pathway (CP) for stroke care local network is almost totally unified and it is well used, but the entry way from acute stage hospitals is only provided by 3 hospitals. Also, the number of pass through cases remains steady, because in Sapporo, many acute stage hospitals have a kaifukuki rehabilitation ward. The present CP uses a cycloid type area cooperation pass that is important in recurrence prevention. Its use began last year in Hokkaido, mainly around Sapporo. When we use this system, we have to input a large amount of patient data, which can be very burdensome. However, its use will continue to expand in the future.
