Abstract : A standardized critical pathway has been used in almost all regional stroke care networks in Kanagawa prefecture for the last four years. This has been accomplished through the cooperation of hospitals, administrative organs, medical associations, and care manager associations in order to improve the system of seamless care for stroke patients transferring from acute hospitals to convalescent rehabilitation wards and then further on to community support clinics. Currently, 16 networks use this common critical pathway in Kanagawa prefecture. The authors belong to the Meeting of Western Kanagawa Stroke Care Network. This network consists of ten acute hospitals and nine convalescent rehabilitation wards and 103 community support clinics. Some 1229 critical pathways were used in this network during the past four years and over 525 were received from other networks in convalescent rehabilitation wards during the same period. This fact strongly supports this common critical pathway's wide use. However, the usage rate of this critical pathway in acute hospital was only 17%. It seems that 83% of stroke patients did not transfer to convalescent rehabilitation wards. Also, the 80% of the stroke patients admitted with critical pathways to convalescent rehabilitation wards returned home. It is possible that some critical pathways are used from convalescent rehabilitation wards to community support clinics and/or home care support service. We should therefore establish a stroke care network from acute hospitals to maintenance hospitals and community support clinics more widely.
