リハビリテーション科医の果たすべき役割

The Role of a Physiatrist

はじめに

　我が国では今後労働人口の減少と後期高齢者数の増加が予測されている．75歳前後から認知症発症率，要介護認定率および医療費が増加することが示されており，後期高齢者の増加は医療・介護ニーズの増大に直結する．一方，高齢者を支えるべき労働人口の減少は，医療・介護に従事する労働者と税収の減少による財源不足があいまって社会全体の問題となりうる．このような状況下で，地域リハビリテーション（以下，リハ）の目標である「障害者，高齢者が安心して暮らし続けることができる地域社会」を実現することは，相当に困難な活動であるといえる．

　2010年3月，地域包括ケア研究会から報告書^1）が発表された．それによると，団塊の世代が75歳以上となり高齢化が急速に進む2025年には，地域包括ケアシステムが構築されていることが必要であるとされた．地域包括ケアシステムとは「概ね中学校区単位で，個々人のニーズに応じて，365日24時間，医療・介護等の様々なサービスが適切に提供できるような地域での体制」であり，そのコーディネーターは地域包括支援センター（以下，包括C）とされた．

　地域包括ケアの実現は我が国の将来のために極めて重要な課題である．具体的には，年齢に関係なく障害を持っても安心して暮らすことができるようなまちの実現であると考えるが，その実現には住民（当事者）や専門職・組織が力を合わせて行う地域リハ活動が必須である．

Abstract : In Japan, a reduction in the productive population and an increase in the number of elderly people are widely predicted for the future. Therefore, building a comprehensive community care plan by 2025, when aging will advance quickly, is urgently required. The coordination of medical treatment, patient care, and resident activity, all provided in a small area, will be important. Furthermore, cooperation amongst a wide range of hospitals and home care providers is also important. Then, after systematizing a hospital network and a home care provider network in a broad region, it will be necessary to undertake deliberations to define the level and type of cooperation required. In order to create an environment for such deliberations, an organization which coordinates services for the whole region, including medical treatment and care, is indispensable. Since there are not many physiatrists in Japan, they need to work creatively so that their impact will be made broadly. It is therefore desirable for physiatrists to be members of these regional coordinator groups, where they can best demonstrate the capability of rehabilitation team coordinators.
