Abstract : In Japan, a reduction in the productive population and an increase in the number of elderly people are widely predicted for the future. Therefore, building a comprehensive community care plan by 2025, when aging will advance quickly, is urgently required. The coordination of medical treatment, patient care, and resident activity, all provided in a small area, will be important. Furthermore, cooperation amongst a wide range of hospitals and home care providers is also important. Then, after systematizing a hospital network and a home care provider network in a broad region, it will be necessary to undertake deliberations to define the level and type of cooperation required. In order to create an environment for such deliberations, an organization which coordinates services for the whole region, including medical treatment and care, is indispensable. Since there are not many physiatrists in Japan, they need to work creatively so that their impact will be made broadly. It is therefore desirable for physiatrists to be members of these regional coordinator groups, where they can best demonstrate the capability of rehabilitation team coordinators.
