子宮内膜症性癒着に対するfinger assist techniqueの導入

Adhesiotomy for severe pelvic adhesion of endometriosis by the finger assist technique

　婦人科疾患のなかでも強固な癒着を伴う子宮内膜症は，特有な疼痛で患者を苦しめる．子宮内膜症の腹腔鏡下手術ではその癒着への対処が手術の鍵を握っている．しかし鉗子操作や電気メスなどでの剥離には技術的な制約があり，腸管や尿管などの臓器損傷や出血などの危険性がつきまとう．そこでわれわれは，安心して剥離操作ができる方法として指の触覚を導入したfinger assist tech-niqueを考案，開発した．今後，子宮内膜症の手術適応を大きく広げるものと考える．

Many women suffer from dysmenorrhea due to pelvic adhesion of endometriosis. The key of laparoscopic op-eration for endometriosis depends on adhesiolysis of severe adhesion. Adhesiotomy by forceps and electrocautery requires a great deal of skill and experience, and there is risk of bleeding and injury of ureter and intestine. We have developed a safe and effective technique of adhesiotomy for severe adhesion of endometriosis. We call this the “finger assist technique”.

We believe that this technique will allow us to expand the indication of operation for endometriosis.
