腹腔鏡補助下幽門側胃切除後空腸パウチ間置術

Laparoscopy-assisted distal gastrectomy with jejunal pouch interposition for gastric carcinoma

　腹腔鏡下手技の進歩により腹腔鏡補助下幽門側胃切除術が普及しつつあるが，再建で空腸パウチを付加する手技は確立されていない．われわれは2／3以上の切除範囲を有する7例の早期胃癌に対し，腹腔鏡下幽門側胃切除を施行し，再建を小開腹創から空腸パウチを作製し間置した．パウチ長は約10cm，導管は約5cmとした．パウチ間置にあたり，必要な腸管のみを創外に引き出し，その他は完全に腹腔内に還納しておくことで6〜7cmの小開腹で十分手術可能である．術中・術後に合併症はみられなかった．腹腔鏡補助下にパウチ間置を行うことにより術後早期のQOLと長期のQOLの改善が得られ，安全で有効な術式と考える．

Although laparoscopy-assisted distal gastrectomy has become a common surgical procedure recently, the techniques with jejunal pouch interposition after laparoscopic distal gastrectomy has not been well established yet. We performed laparoscopy-assisted distal gastrectomy with jejunal pouch interposition constructed through the mini laparotomy on 7 patients with early gastric cancer that required resection of more than 2/3 of the stomach.
