胸部刺創緊急手術における胸腔鏡の役割—手術アプローチ法の決定に関して

Efficacy of thoracoscopy for the decision of surgical approach in emergency surgery for chest stab wound

　胸部刺創の緊急手術において，手術アプローチ法の決定に関して胸腔鏡が有用であるか否かについて検討した．胸腔内に達する胸部刺創で緊急手術となった5症例の手術のはじめに胸腔鏡観察を行った．胸腔鏡にて得られた情報量は症例により異なるが，全例で有効なアプローチ法を選択することができた．胸腔鏡により十分な観察ができた場合には最適な手術アプローチ法が決定できる．特に出血の少ない場合には有用である．出血量が多く，広範な胸腔内血腫を伴っている場合には，胸腔鏡による正確な損傷部位の診断は困難である．しかしその場合でも，心膜と横隔膜の損傷の有無などの有用な情報が得られる可能性がある．

The purpose of this study was to assess the efficacy of thoracoscopy for the decision of surgical approach in emergency surgery for chest stab wounded patients. Five patients underwent thoracoscopy following emer-gency surgery. Although information through thoracoscopy was varied, an effective surgical approach could be selected in each case. When thoracoscopic observation is sufficient, decision of effective surgical approach is easy, especially if bleeding is minimal.
