胸腔鏡視下脊柱変形前方矯正固定術

A new system using thoracoscopic approach for anterior correction and fixation for scoliosis

　内視鏡下に側彎症に対する前方矯正固定術を行えないかと考え，1994年より研究開発を開始した．アウトリガーを応用したシステムとして，1995年から日本の医療器械メーカーと共同で開発を進めた．胸壁付きの豚屍体胸椎を用いての手術を行い，続いて胸腔鏡視下に麻酔下の豚の手術を繰り返し行い，システムの開発・改良を行った．その結果，1999年より臨床応用を開始した（体外矯正・体内固定術：external correction ＆ internal fixation system，ECIFシステム）．本法を用いた場合，初期平均側彎矯正率は80％以上と強力であり，腋窩線上の少数の15〜20 mmのポート創のみで手術が行え，術後疼痛の軽減，術後回復が早いなどの利点がある．

Purpose: In 1994, we have started research and development of a new system for the thoracoscopic anterior correction and fixation of spinal deformities, External Correction and Internal Fixation (ECIF) system. The aim is to correct scoliosis using an outrigger apparatus and to fix the spinal column with the rod and screw system. After animal experimental surgery (Spine 25: 876-83, 2000), we have started its clinical applications from 1999.
