縦隔気管支嚢腫に対する胸腔鏡下手術

Thoracoscopic surgery for bronchogenic cyst of the mediastinum

　1994年から2000年までに胸腔鏡下手術を施行した縦隔気管支嚢腫の10例について検討した．対象は男性4例，女性6例，年齢は26〜55歳（平均41.4歳）であった．腫瘍部位は上縦隔1例，中縦隔1例，後縦隔8例で，腫瘍径は2〜6cm（平均3.5cm）であった．術前診断および周囲臓器との癒着の予測にはMRIが有用であった．術式は胸腔鏡下手術が6例，ミニ開胸併用が4例であった．手術時間は55〜170分（平均95分），胸腔ドレーン留置期間は1〜7日（平均2.8日），術後在院日数は5〜15日（平均10.2日）であった．術後合併症は認めなかった．縦隔気管支嚢腫に対する胸腔鏡下手術は低侵襲で安全な手術であるが，状況によってミニ開胸を併用すべきである．

We retrospectively reviewed 10 patients of mediastinal bronchogenic cyst who had undergone video-assisted thoracic surgery (VATS) between 1994 and 2000. There were 4 males and 6 females. The mean age was 41.4 years (range, 26-55 years). One tumor was located in the superior, 1 in the middle, and 8 in the posterior mediastinum. The mean tumor diameter was 3.5 cm (range, 2-6 cm). MRI findings were useful in the preopera-tive diagnosis and the estimate of the adhesion with the surrounding organs.
