胸腔鏡下縦隔腫瘍切除術における視野確保のための工夫

A technical tip for thoracoscopic resection of mediastinal tumor

縦隔腫瘍に対しては，胸腔鏡下手術はその低侵襲性を考慮するとよい適応であるが，しばしば手術に難渋する症例を経験する．その際，腫瘍の存在部位，大きさにより，視野確保が不十分と感じることも多い．前縦隔病変に対しては視野確保のために胸骨吊り上げ法などの工夫がなされているが，腫瘍が横隔膜，食道，心臓に囲まれた部位に存在する後縦隔病変に対しても視野確保のための有効な方法，手術器具の必要性を感じることも多い．今回，後縦隔腫瘍に対して胸腔鏡下切除を行う際に，分散型多方向性ハートポジショナーTentacles^Ⓡ（住友ベークライド）を用いて，良好な視野確保が可能であった症例を経験したので報告する．

　Considering the less invasiveness, thoracoscopic resection is a suitable procedure for treating mediastinal tumor. However, we have experienced difficulties in exposing the tumor in some cases because of the location and the size of the tumor. At those instances, we often thought a new method or device for exposing the tumor would be necessary. We introduce our experience of a patient with post mediastinal tumor that was resected by thoracoscope using Tentacles^Ⓡ, a commercially available device that is designed to lift the heart during off-pump CABG. This device was very useful for exposing the post mediastinal tumor.
