胸腔鏡下前・中縦隔腫瘍摘出術

Thoracoscopic resection for anterior and middle mediastinal tumor.

　前・中縦隔は縦隔腫瘍の好発部位であり，胸腺腫，奇形腫，嚢胞性疾患などが発生する．縦隔腫瘍で最も頻度の高い胸腺腫は、たとえI期でも術前には病期の確定が困難であり，また再発例が報告されていることより胸腔鏡下手術の適応はない．リンパ性腫瘍，未熟奇形腫，胚細胞性腫瘍も診断的胸腔鏡手術の適応はあるが，摘出術としての適応はない．胸腔鏡手術の適応は、気管支性嚢胞や心膜嚢胞，胸腺嚢胞などの先天性嚢胞や成熟奇形腫などの良性縦隔腫瘍が良い適応である．手術操作も剥離が主体であり，手技的にも容易で，従来の開胸操作に比べて利点の多い手術である．

Mediastinal tumors such as thymoma, teratoma and congenital cystic diseases often occur from anterior and middle mediastinum. Thymoma which is most popular mediastinal tumor is contra-indication for thora-coscopic surgery. It is difficult to determine the stage of thymoma preoperatively and to resect the tumor as extended thymo-thymectomy thoracoscopically. Im-mature teratoma, germ cell tumor and lymphoma are also contra-indication for thoracoscopic resection. But thoracoscopic diagnostic biopsy is good indication for these tumors. Congenital cystic diseases and mature teratoma are good indication for thoracoscopic surgery.
