Liver Hanging Maneuverを用いた腹腔鏡補助下肝右葉切除術

Laparoscopy-assisted Right Hepatic Lobectomy Employing the Liver Hanging Maneuver

　腹腔鏡下肝切除術は外側区域切除術あるいは肝辺縁の部分切除術が行われているものの，肝葉切除術はまだ一般的ではない．安全かつ確実に肝葉切除術を施行できればその有用性に疑問はないが，主に癌を対象とするため十分なsurgical marginを確保した正確な肝離断が必要で，出血コントロールなどの安全面も考慮しなければならない．われわれは，下大静脈前面を露出してから短肝静脈を直視下に処理可能なliver hanging maneuverを用いて腹腔鏡補助下肝右葉切除術を施行した．肝離断面の展開も容易で，中肝静脈右壁を露出させた正確な肝離断が可能であった．手術時間，出血量，surgical marginは開腹手術と比べて遜色のない成績であった．

Laparoscopy has often been used to perform lateral segmentectomy and partial resection of the liver margin, but not hepatic lobectomy. There is no question that laparoscopic hepatic lobectomy is very useful, even in sur-gery for cancer patients, provided that hepatic lobectomy is performed safely and accurately. Therefore, ensur-ing sufficient surgical margin and adequate hemorrhage control during laparoscopic procedures are important.
