◆要旨：［目的］近年，腹腔内臓器把持用の機器として，Internal Organ Retractor^Ⓡ（IOR）が開発された．今回IORを牽引用具として用いた腹腔鏡補助下大腸切除術（LAC）の有用性に関して検討を行った．［対象］2015年1月〜12月に当院で施行した，上下部直腸癌を除いたLAC症例45例中，IORを使用した20例と細径鉗子を使用した25例において術後早期経過に関して比較・検討を行った．［結果］手術時間，出血量，リンパ節郭清個数，食事開始および排便までの日数，術後在院日数のすべてにおいて，両群間で統計学的な有意差は認めなかった．［結論］IORを使用したLACは，ポートや細径鉗子を追加することなく術野展開が可能で有用な手技と考えられた．

［Purpose］Recently, the internal organ retractor^Ⓡ（IOR） has been developed to hold organs in the abdominal cavity, and we are using this instrument for laparoscopy-assisted colectomy（LAC）.［Method］ Between January and December 2015, 45 patients underwent LAC in our hospital. IOR was used in 20 patients, and needle forceps used in 25 patients. We compared short term surgical outcomes between the two groups.［Result］ There appeared to be no significant difference in operative time, blood loss, the number of lymph nodes dissection and postoperative days of defecation, diet and hospital stay between the two groups.［Conclusion］ This study showed LAC with IOR is useful.
