3か月間で急速な拡大を示した胸腺嚢腫の胸腔鏡下手術の1例

Thoracoscopic Surgery of a Thymic Cyst that Rapidly Enlarged over a 3-month Period ; A Case Report

　胸腺嚢腫が短期間で拡大傾向を示すことは稀である．3か月間で急速に拡大を示した高齢者胸腺嚢腫の1例に対して，胸腔鏡による摘出手術を行ったので報告する．患者は79歳，女性．狭心症で通院加療中だった．2001年5月までの胸部X線では異常影は認めなかった．同年9月に胸部圧迫感を訴え，胸部X線で左側に縦隔腫瘍を疑われたため，当科に入院となった．胸部CTやMRIで，腫瘍は嚢腫であると考えられた．手術は，胸腔鏡下に腫瘍切除を行った．病理組織学的にも胸腺嚢腫と診断され，急速拡大の原因は嚢胞内への出血が考えられた．心疾患合併高齢者に対して胸腔鏡下に手術を行うことによって，創部痛の軽減や早期の離床と退院が可能であった．

We report a case in which total thoracoscopic surgery was performed to treat a thymic cyst that rapidly en-larged over a 3 month period in an elderly patient.The patient was a 79-year-old woman who was attending the outpatient clinic for angina pectoris and in whom no abnormal shadows had been detected on chest x-rays until May 2001.In September of the same year,a mediastinal tumor was suspected,and she was admitted to our department.CT of the chest showed a 8×5 cm mass-like shadow in the left anterior mediastinum.On MRI,it was high intensity on a T2 weighted image and was thought to represent a cyst.
