血液透析患者の肩部痛に対する日帰り内視鏡下手術—15年間にわたる治療経験

Day Surgery using Endoscopic Management for Shoulder Pain in Long-term Haemodialysis Patients

　われわれは，長期血液透析患者における肩部痛の原因を透析アミロイドーシスによる肩峰下腔および結節間溝部分の相対的狭小化による肩インピンジメント症候群と考え，1987年から内視鏡下手術を行ってきた．本稿では，本術式の長期成績について報告した．対象は，手術後6か月以上の経過観察が可能であった478肩である．手術は局所麻酔下に日帰り手術とし，universal subcuta-neous endoscope（USE）systemとヘルニア鉗子を用いて，鳥口肩峰靭帯と肩峰下滑液包，横上腕靭帯と上腕二頭筋滑液鞘の切除を行った．肩部痛は破壊性頸椎症や腱板損傷合併例以外では消失した．肩関節可動域は，術前に比べて術後は有意に改善した．本法は，透析患者の肩部痛の治療として有効な手術法である．

Some long-term haemodyalysis patients suffered from shoulder pain during haemodialysis, while sleeping, and during recovery when sitting and standing position. We think these patients are suffering from shoulder im-pingement syndrome due to haemodyalytic amyloidosis, and recommend endoscopic surgery. Under local anes-thesia with 1.5% lidocaine 15ml, we performed resections of the coracoacromial ligament, subacromial bursa, transverse humeral ligament, and synovial sheath of the biceps long head tendon in 478 shoulders.
