術中イレウス管造影が有効であった癒着性イレウスに対する腹腔鏡（補助）下手術症例

Laparoscope-assisted Operation for Adhesive Ileus with Effective Intraoperative Contrast Imaging

　われわれは，術後癒着性イレウスに対し，腹腔鏡下手術を試みている．今回，腹腔鏡（補助）下イレウス解除術の際，ガストログラフィンを用いた術中イレウス管による透視下小腸造影（術中イレウス管造影）が，手術操作中の腸管損傷予防や腸管閉鎖部位，術直後の通過状況の確認などにおいて有用であった2症例を報告する．腹腔鏡による腹腔内観察に加え，術中イレウス管造影により情報が集積され，腹腔鏡下手術の確実性が増し，その適応は拡がるものと思われた．

We perform laparoscope-assisted operation for adhesive ileus. Here, we report two cases in which we per- formed laparoscope-assisted operation to release ileus with intraoperative contrast imaging. Gastrografin was used as contrast medium. This technique was very useful because it prevented from damaging the intestine during the operation. It also helped confirm the obstructive site and the post operative condition of the passage. This procedure allows us to obtain more information and thus, reliability and indication for laparoscope-assisted operation may further be increased.
