腹腔鏡下胆嚢摘出術におけるLapro-Clipの使用経験

The Use of Lapro-Clip in Laparoscopic Cholecystectomy

　今回，われわれは腹腔鏡下胆嚢摘出術において，胆嚢胆嚢動脈の体内残存側に，吸収性クリップのLapro-Clipを使用し，良好な結果を得たので報告する．クリッピングは結紮物に応じて，幅8mmあるいは12mmのLapro-Clipを1個使用した．胆嚢動脈はすべて幅8mmで処理し，慢性胆嚢炎症例の胆嚢管処理のみ12mm幅を6例に使用した．Lapro-Clipは金属クリップと比較して，引き抜き抵抗からみてもクリップ力は高いものと考えられ，出血や胆汁漏などの合併症は認めず，クリップ1個での処理が可能であった．また，胆管周辺の異物残存は極力避けるべきであり，吸収性のLapro-Clipは有用と考えられた．

An absorbable surgical ligating clip (Lapro-Clip, Davis & Geck, USA) was used in 29 patients who underwent laparoscopic cholecystectomy. In this study, Lapro-Clips were routinely used for ligation of the cystic duct stump and artery. A 12 mm ligating clip was used to ligate the cystic duct for 6 patients with chronic inflammation. Lapro-clip, an 8 mm ligating clip was used for all the remaining patients. Consider-ing the force required to dislodge the clips, Lapro-Clip showed significantly greater attachment than the metallic clips.
