大腿輪に嵌入した再発鼠径部膀胱ヘルニアに対して腹腔鏡下に修復しえた1例

A case of laparoscopic repair of femoral hernia involving the bladder

◆要旨：症例は70歳，男性．右鼠径部膨隆を主訴に当院を受診した．9年前に右鼠径ヘルニアに対してメッシュプラグ法による修復術の既往があった．腹部単純CT検査で再発右鼠径部膀胱ヘルニアと診断し，transabdominal preperitoneal repair（TAPP）法で修復を施行した．鼠径ヘルニアは認めず，膀胱が大腿輪に嵌入しており，大腿輪をヘルニア門とした膀胱ヘルニアと診断した．膀胱を損傷しないように剝離を進め，膀胱，ヘルニア囊を還納した．術後経過は良好で術後第2病日に退院となった．術後半年経過した現在，ヘルニアの再発はなく，排尿障害も改善している．今回われわれは大腿輪に嵌入した膀胱ヘルニアに対して腹腔鏡下に修復しえた1例を経験したため，若干の文献的考察を加えて報告する．

　The patient is a 70-year-old man. He presented to our hospital with a chief complaint of right inguinal bulging. Nine years ago, he underwent a mesh plug repair of a right inguinal hernia. Abdominal computed tomography（CT） showed a recurrent right inguinal bladder hernia, and we decided to perform transabdominal preperitoneal repair（TAPP）. Intraoperative findings revealed no inguinal hernia, and the bladder herniated into the femoral ring. A diagnosis of femoral hernia involving the bladder was made. The preperitoneal space was dissected to avoid damaging the bladder, and the bladder and hernia sac were reduced, and the hernia orifice was repaired using mesh. The postoperative course was uneventful, and the patient was discharged on the second postoperative day. Six months after the surgery, there has been no recurrence of hernia and no evidence of dysuria.
