腹腔鏡下鼠径ヘルニア修復術後の腹膜閉鎖間隙に虫垂嵌入を認めた1例

A Case of Appendix Incarceration Following Laparoscopic Hernia Repair

　腹腔鏡下鼠径ヘルニア修復術後の虫垂の嵌入をきたした症例を経験した．症例は66歳，男性．右鼠径ヘルニアに対して腹腔鏡下鼠径ヘルニア修復術（経腹膜的アプローチ）後，1年6か月目に左側の鼠径ヘルニアを認め来院，腹腔鏡下手術を再度施行した．手術時，虫垂が前回手術時の腹膜欠損部に嵌入していた．嵌入虫垂を剥離し，右トロッカーより体外に誘導し虫垂切除を行った．再度トロッカーを挿入し，左鼠径ヘルニア修復術を行った．本症例は腹腔鏡下鼠径ヘルニア修復術において稀な併発症と考えられたので報告した．

An 66-year-old man received laparoscopic hernia repair (transabdominal preperitoneal repair) for right inguinal hernia in March 1995. After 1 year 6 months, he recognized left inguinal swelling, and consulted our hospital. Laparoscopy revealed that the repaired peritoneum using the hernia stapler was separated, and the appendix was incarcerated and adhered to the hernia mesh. We exfoliated and dissected the appendix under laparoscopic procedure. The appendix incarcerated to the hernia mesh is a very rare complication, which never occurs in conventional hernia repair.
