右頸部気縦隔アプローチを併用した非胸腔アプローチによる食道癌根治術

Mediastinoscopic radical esophagectomy with right cervical approach under pneumomediastinum

◆要旨：筆者らは食道癌根治術として十分な縦隔郭清を食道裂孔および頸部から縦隔鏡下に行う手術手技を臨床応用するためThiel法固定献体を用いて検討・開発し，報告してきた．左頸部気縦隔法のみでも106tbLリンパ節の郭清が可能であり，臨床例でも一部応用しているが105および106recRの郭清は一部不十分であることが判明した．そのために右頸部からも気縦隔操作を併用したところ，より徹底した上縦隔郭清が施行可能であることが分かり，ここにその手技を報告する．

We previously reported the performance of “mediastinoscopic esophagectomy with lymph node dissection”（MELD） under pneumomediastinum using a transcervical and transhiatal approach, as a method of radical esophagectomy. A part of this procedure are performed in clinical cases as clinical trials. We revealed that the 106tbL lymph nodes were almost completely retrieved but the upper thoracic paraesophageal lymph nodes（105） and the right recurrent nerve lymph nodes（106recR） were not completely retrieved. By adding the right cervical approach in pneumomediastinum, a more complete dissection of these lymph nodes could be performed and herein we reported this procedure.
