腹腔鏡下直腸切断術における腹膜外経路人工肛門造設術の工夫—15mmトロッカーを使用して

Extraperitoneal colostomy in laparoscopic abdominoperineal resection using a laparoscopic trocar

◆要旨：直腸切断術において傍ストーマヘルニア，ストーマ脱出，イレウス予防目的に腹膜外経路のストーマ造設が行われてきたが，腹腔鏡下直腸切断術においては手技的な困難性から腹腔内経路が選択されることが多い．今回筆者らは腹腔鏡下直腸切断術において，15mmトロッカーを使用して腹膜外経路を作製しストーマを造設する方法を5例に施行した．本術式は手技が簡便であり，腹腔鏡下に腹膜外経路で人工肛門を造設する手技として有用な方法と思われる．

Extraperitoneal route is considered to be effective to prevent parastomal hernia, stoma prolapse, and ileus following abdominoperineal resection, but it has not been applied laparoscopically because of its technical difficulties. We performed extraperitoneal colostomy for five cases of laparoscopic abdominoperineal resection using a 15mm laparoscopic trocar. This procedure is easy and useful for laparoscopic stoma creation through the extraperitoneal route.
