内側乳癌症例に対し単孔式内視鏡下乳房温存術を施行した1例

Single-incision endoscopic breast conserving surgery in a patient with internal side breast cancer

◆要旨：単孔式内視鏡下乳房温存術を施行した内側乳癌症例を経験したので報告する．患者は46歳女性で，右乳腺腫瘤を主訴に当科を受診した．右乳腺AB領域に2cmの腫瘤を触知した．マンモグラフィでは境界明瞭でやや高濃度の円形腫瘤を，超音波では境界が比較的明瞭な低エコー腫瘤を認めた．穿刺吸引細胞診は乳癌の診断であった．手術時間は180分，出血量は130mlであった．術後良好で,第5病日に退院した．本術式は整容性に優れており，内側乳癌症例に対する単孔式内視鏡下乳房温存術は有効な術式と考える．

　We report a case of a internal side breast cancer which was successfully resected by single-incision endoscopic breast conserving surgery. A 46-year-old woman was referred to our hospital because of a right breast tumor. Mammography showed a dense well-defined category 4 tumor. Ultrasonography showed a round heterogenous hypoechoic tumor. Fine-needle aspiration cytology indicated carcinoma. The operative time was 180 minutes and intraoperative blood loss was 130ml. The patient was discharged on postoperative day 5 without operative complications. This technique contributed to the improvement of the patient's cosmetic effect. The single-incision endoscopic breast conserving surgery for internal side breast cancer may be considered as a useful operative method.
