◆要旨:本邦では胸腔鏡下食道切除術の胸部操作は,従来の手術と同様に左側臥位で行われるのが主流である.左側臥位での胸腔鏡下食道切除術では助手およびスコピストの習熟した術野展開・確保の技術が要求されるが,腹臥位での胸腔鏡下食道切除ではこれらの問題点が少ないと報告されている.当科における腹臥位胸腔鏡下食道切除術の導入,術前準備さらに術中・術後の管理について報告する.
Thoracoscopy assisted esophagectomy in the left lateral decubitus position has been performed regularly as a standard method for esophagectomy in Japan. Recently, prone positioning during thoracoscopic esophagectomy has been reported to eliminate the need for a technically skilled assistant and camera surgeon in the left lateral decubitus position. Herein, we reported the evaluations of induction to our department, preoperative preparations and intra-/postoperative findings of thoracoscopy assisted esophagectomy with the patient in the prone position.
