腹腔鏡下に修復した後区域枝肝管損傷の1例

Laparoscopic repair of an aberrant right posterior sectoral hepatic duct injury

◆要旨：患者は77歳，男性．胆石症の診断にて，腹腔鏡下胆囊摘出術を施行した．術中に後区域枝肝管を約3分の1周にわたり損傷した．そのため，径5mmのドレナージチューブを損傷部より留置した．また，胆管内の減圧を目的にCチューブを挿入してドレナージとした．術後は特に問題なく経過し，術後8日目にCチューブを抜去した．本症例では術前に通常とは異なる胆管の分岐形態が確認されており，注意して操作を行うべきであった．胆管損傷に対してステント留置および減圧のためのCチューブ留置は有用であった．

　The patient was a 77-year-old man who was diagnosed with cholelithiasis and underwent laparoscopic cholecystectomy. During the operation, we injured an aberrant right posterior sectral hepatic duct. We placed a 5 mm drainage stent tube laparoscopically into the injured hepatic duct, and performed C-tube decompression. On the eighth postoperative day, we removed the C-tube. Postoperative course was good. Preoperative examination showed an anomaly in the bile duct, so careful attention was required. Drainage stent tube and C-tube were useful for bile duct injury.
